Design Medicaid navigation around access

A useful pathway goes beyond a referral. It checks whether an option is clinically appropriate, available, covered for that member, and realistically reachable. Teams should measure failures at every handoff and retain an explicit route for escalation.

Start with actual service availability

Select a defined service area and verify destination hours and acceptance. A list of providers does not establish access. Decide how member services should respond when no suitable option is available and how that exception will be recorded.

Make transport an explicit workstream

NeXtriage’s application includes transport sponsorship, eligibility, fulfillment, and trip-state concepts. These are implementation capabilities to evaluate, not a promise that every plan or geography is covered. Confirm the payer arrangement, provider connection, and cancellation process.

Handle coverage and identity exceptions

Map what happens when roster data are old or identity cannot be matched. A failed match should prompt a defined support workflow. It should not silently become a statement that the person lacks coverage or should defer care.

Evaluate access alongside utilization

Measure offered options, acceptance, completion, cancellation reasons, and member experience. Review differences by service area and access needs using privacy-preserving reporting. A lower emergency department count is insufficient evidence if people were unable to obtain needed care.
